A) Asking:

She has suffered from.fullness abdominal for 3 months

All the time she felt distention of her abdominal.

When she desire defication but can not do completely. Feel still somting inside with uncomfortable..Also she feel hot sensation of her face.

Her sleep is turibid (wake up 2~3times during sleep)

Prefer to a lot of warm water.

The symptom is relieved by rest, drinking warm water and takes less food.

Present Symptoms:

: Good appetite, no chill & fever, no abnormal sweating, no thirsty (prefers warm water), stool (once per day, with form, hard push out), normal urination, easy to feel tired,

Past history:

12 years ago she got injury of her neck( c-5,6,7)

Life Style: No smoking, house wife

Family history: NAD

Menstruation, marital & obstetrical history / Men’s sexual health

Menopause( 4 years ago) married with 3 children

No limitation of movement.

Case Summary

Definition:

Abdominal distention refers to distention and fullness which occur in both the upper and lower abdomen. The stomach and spleen are located in the upper abdomen, while the small and large intestines are in the lower. Dysfunctions of these four organs may lead to abdominal distention.

WMS:

Abdominal Distention is the actual “physical” finding that the abdominal area is larger than normal. It can be caused by three different things: air, fluid and tissue.

There are continuous and intermittent distentions.

Continuous distention is swelling of the abdomen for a considerable period of time, with no signs of relief. The cause can be one of four things.

· Enlargement of an organ within the abdomen

· A tumor in the abdomen

· An increase of fluid around the organs in the abdomen

· Obesity

Intermittent distention can come and go and is usually triggered by ailments which inhibit proper digestion, and cause an accumulation of fluid or gas within the digestive system.

Distention Causes

One common cause is irregular digestion. When the digestive system is working properly, food is digested by chemical reactions and broken down into components. From these components the body obtains nutrients and energy to use as fuel.

Food that is not broken down is considered waste and is eliminated from the body. Some of the waste comes in the form of odorless gases and are easily handled by the intestinal canal.

When the body is not digesting properly, sulfur-bearing gases are produced and accumulate in large pockets. These gases stretch the intestinal canal producing the painful effect of bloating.

TCM Aetiology and Pathology

According to TCM, there are three causes that lead to Fullness abdomonal

(1) Irregular or excessive food intake impairs the stomach and intestines, causing dysfunction of transportation and transformation, thus the retained food is stagnated and blocks the flow of Qi, causing abdominal distention.

(2) Because of the weakened function of the spleen and stomach or ageing, long illness, and general debility, the spleen and stomach fail in their transporting functions, so the circulation of Qi in the stomach and intestines is impaired, resulting in abdominal distention.

(3) Abdominal operations may disturb the circulation of Qi in stomach and intestines, leading to abdominal distention.

Abdominal distention is common in clinic.Distention and fullness are likely to occur in both the upper and lower abdomen.The ST is located in the upper abdomen,while the small and large intestines are in ,they jointly complete the stroge, Qi Def. of the SP and ST results in dysfunction of transportation and transformation.In this patientc symptoms are when she desire defication but can not do completely. Feel still somting inside with uncomfortable. Its related transportation and transformation function disorder of the spleen and the stomach

Tianshu:restors and harmonizes the flow of Qi in large intestine and desolve stagnation

Life Style Advice

l Massage around low abdominal area make circle right to left.

l Take meal regulary

l Have regular exercises. Movement, especially in the morning before breakfast, encourages bowel activity

l Eat natural fresh foods. Packaged foods contain more preservatives and are harder to digest.

l Avoid foods that are too cold or too hot. They increase the air intake during a meal.

l Avoid caffeine. It acts as a stimulate and can overexcite the digestive system.

Case Conclusion

The patient was KIWI people. I am very worried with this patient with communication for diagnosis.

But it’s no problem because she know well Chinese medicines even acupuncture. She have experience with other student in our school..she know acupuncture is very effective for humen disease.

The points and manupulationmethods I used for this patient are proved to be helpful to the illness because the patient does not came for 2nd treatment. It’s means fully recoverd her symptoms. So I would apply them to other patients again.

A) Asking

The patient suffers from discomfort in epigastria for 10 days. There was some problem came up two weeks ago and the patient was worried and stressed about it. Then one day, it suddenly appeared after she had a dinner with severe cramping pain in the stomach. She had nausea and few times of vomiting. After the cramping pain has been relieved, she started having poor digestion and bloated sensation after eating. No aggravating factors for epigastric pain. Frequent belching and acid regurgitation present.

No chill. No fever. Dry mouth and throat. Hard to fall asleep (sleeping well otherwise). Poor appetite. Normal urination. Fatigue. Constipated for 3 days. (Stool is usually loose.) Depressed.

Life style: Busy, stressed, likes coffee

C) Explanation

Anxiety, anger and mental stress damage the liver, causing failure of the liver in dominating free flow of qi, adversely attacking the stomach, impeding its activity and hindering its qi descending, then pain appears. Without any unusual food intake, the epigastric could possibly be caused by emotional stress that she has been suffering so far. For the last few weeks, the patient has gone through emotionally hard situation and I personally think that this problem has been developed due to her mental stress and anxiety. Once stagnant liver qi attacked the stomach, she started getting frequent belching, acid regurgitation, and poor appetite, bloated sensation after eating and poor digestion. Also due to anxiety and stress, she has difficulty in falling asleep too. Pink dim tongue body with white coating and taut weak pulse also indicate weak middle jiao qi and interior syndrome due to emotional factor.

EPIGASTRIC PAIN

TCM DEFINITION

Epigastric pain refers to a syndrome manifested by frequent pain over the epigastric region and close to the cardia, so in ancient times, it was also named “cardiac pain” or “cardioabdominal pain”. It is commonly seen in acute and chronic gastritis, stomach or duodenal ulcer, functional stomach pain in modern medicine.

TCM ETIOLOGY and PATHOGENESIS

(1) Irregular food intake, raw and cold food and hunger injure the spleen and stomach, causing failure of the spleen in transportation and transformation and failure of the stomach qi in descending, then pain appears.

(2) Anxiety, anger and mental depression damage the liver, causing failure of the liver in dominating free flow of qi, adversely attacking the stomach, impeding its activity and hindering its qi descending, then pain appears.

(3) Generally, lowered functioning of the spleen and stomach, due to invasion of pathogenic cold, which is stagnated in the stomach, causes failure of the stomach qi in descending, then pain occurs.

DIFFERENTIATION

1. Food retention

Clinical manifestation

Distending pain in the epigastria, aggravated on pressure, belching with fetid odour, anorexia, pain aggravated after meals, thick sticky tongue coating, deep forceful or rolling pulse.

2. Attack of the stomach by the liver qi

Clinical manifestation

Paroxysmal pain in the epigastria, radiating to the hypochondriac regions, frequent belching, nausea, acid regurgitation, abdominal distension, anorexia, thin white tongue coating, deep taut pulse.

3. Deficiency of the stomach with stagnation of cold

Clinical manifestation

Dull pain in the epigastria, which may be relieved by pressure and warmth, regurgitation of thin fluid, general lassitude, think white tongue coating, deep slow pulse

TREATMENT PRINCIPLE

Clear heat, harmonise middle jiao qi, remove stagnation, calm the mind, warm and tonify SP/ST, remove cold stagnation

ACUPUNCTURE TREATMENT

CASE CONCLUSION

The diagnosis of this case is epigastric pain and the syndrome differentiation is liver qi attacking stomach due to mental stress and anxiety.

I gave her three treatments and at the third visit, most of patient’s symptoms appeared to be improved (only heaviness sensation inside the stomach is remained, appetite has been improved). I have used Xiawan and Neiting with the basic prescription of Zhongwan, Neiguan, Zusanli, Tianshu, Taichong. However it was still difficult for me to resolve her dry mouth and throat by needling.

A. Asking

The patient has suffered from neck and right shoulder pain for 2days. When she does not move, pain is dull but when she moves her shoulder, pain is sharp.

She thinks a reason of pain is bad sleeping position 2days ago. There are no radiating pain, and she does not know what makes it worse or batter.

She hasn't got any injury about shoulder, and she did not use her shoulder too much. She does not feel any pain when I press the shoulder and neck.

No chill and no fever, no headache, no dizziness, good appetite, good sleeping, no dream, normal urination and defecation, no thirsty, no dry mouth, prefer to drink cold water.

Past history : NAD

Lifestyle : 1 soju(眞露)/days, 10 pieces/1 day, stressful life.

Menstruation, Martial & obstetrical history : 13 7/29, serious pain before and during period.

Case summary

Diagnosis : Neck and right shoulder pain.

Treatment principle : remove qi and blood stagnation

Point : Tian zhong, bing feng, ashi point.

Explanation : Since she got this problem, there was no injury or over use of shoulder, just the temperature was so cold, and she slept with bad position, those two thing made qi and blood stagnation on local area.

Bing feng, Tian zhong, ashi point : local point for promoting qi and blood stagnation on local area to relieve pain.

Considerations

The shoulder is the most mobile joint in the human body. The rotator cuff in the shoulder is made up of four tendons. These tendons give the shoulder a wide range of motion.

Any swelling, inflammation, tearing, or bony changes around these tendons causes pain when a person tries to move the arm above the head, behind the back, or straight out in front.

Common Causes

The tendons pass underneath a rigid bony arch in the shoulder. The most common cause of shoulder pain is when the tendons become trapped under this arch. The compressed tendons become inflamed or damaged, a condition called rotator cuff tendinitis. This can occur from general wear and tear as you get older, an activity that requires constant shoulder use like baseball pitching, or an injury.

TCM

The shoulder pain can be caused by obstruction of wind, cold, dampness, phlegm, or qi and blood stagnation. Deficiency of LV and KD, deficiency of Qi and blood, or Qi and blood stagnation can also be the reason of shoulder pain.

The main symptoms are inability to move, such as feel pain when doing back ward rotating, sever pain at night.

There are 4 stages to get recover. Firstly pain in getting sever, secondly pain get reduced but muscle get harder(contract), third, pain all gone but mescle still get contracted, and fourth, muscle get recover and problem all gone.

If the patient have treatment, problem wii gone within 8 months, but if not, it will take 1~2years.

Case conclusion

I treated this patient just once and next day morning, pain was gone. May be the problem is just 2days, short time, so treatment was very effective, I think. Actually I prepared lifestyle advice for her when I get home, but when I woke up in the next morning and she text me she got recover. So I did not put my life style advice in here because lifestyle advice was useless for the patient.

2009 8.8水災 全台單日雨量紀錄大改寫

〔中央社〕民國48年87水災重創台灣中南部13縣市,50年後的8月8日,莫拉克颱風挾帶的超大豪雨更驚人,7、8日兩天就降下台灣山區半年平均雨量,大幅改寫歷年10大單日最大降雨排行榜。

中央氣象局於6日發布莫拉克颱風陸上警報,巧合的是,在颱風警報期間,正好是87水災50周年。莫拉克大雨驚人,氣象局四度上修總雨量預測。氣象局解釋,莫拉克移動緩慢,才會導致降雨量屢屢創新,氣象局也多次上修總雨量預測,從一千六百毫米上修到二千毫米,昨晚更調高到二千五百毫米,這也是氣象局有史以來首次將預估總雨量上修到二千五百毫米。

中 度颱風「莫拉克」在台東、高高屏地區與台南分別傳出淹水與緊急撤村的災情,幾乎可以說是五十年前「八七水災」的翻版。綜觀這兩次相隔了五十年的「八七水 災」,除了在颱風襲台日期上相同外,還有一個共同點:看似天災,但其實都是中國國民黨政權基於政治掛帥、施政罔顧民生所造成的人禍!

五十年 前的「八七水災」,國民黨政權為了「反攻大陸」,硬是要在台灣維持六十萬大軍,致使國防支出占全國總預算的比例一度高達五十%以上(據國民黨政府所發表的 數字顯示,在一九七○年代之前,其比例都在四十%以上,一九七○和一九八○年代都在二十五%以上)。在這種失衡的國家總預算下,必然會排擠到治山防洪所需 的基礎建設,一遇到颱風,重北輕南之下基礎建設不足的南部地區焉能不淹大水?

五十年後的「八七水災」,也是因為國民黨不分青紅皂白地以「有 綁樁之嫌」、「預算浮編」等似是而非的理由阻擋民進黨政府所提出的「八年八百億治水預算」。一直到二○○七年一月,國民黨所控制下的立法院才通過這個預 算,後來甚至加碼到一一六○億元,當時國民黨立委賴士葆甚至提案治水工程全部不必做環境影響評估,很顯然的是國民黨為了奪回政權,不惜以人民為芻狗,成為 一場金權政治與忽視環保的「完美風暴」,盲目的重建,最終讓台灣走向另一場浩劫。

相隔五十年的「八七水災」,除了中國國民黨這個因素之外,台灣人民恐怕也要自我檢討,為什麼會兩度相信這個只以「我黨興亡為己任,置國人死生於度外」的黑心政黨?為什麼會做出這種錯誤的選擇?(作者為公務員)

緊接在莫拉克颱風之後的艾陶颱風10日肆虐日本,豪雨造成中部、西部多處土石流、積水嚴重,已有至少13人喪生。日本氣象廳指艾陶11日將直撲東京,已對全日本發布豪雨和土石流警報,欲前往日本旅遊的民眾應特別注意。

早艾陶一步形成的莫拉克颱風已在菲律賓、台灣和中國造成嚴重災情,菲國有22人喪生。

艾陶颱風帶來強風豪雨,日本兵庫縣佐用町1條河川暴漲決堤,淹沒約480棟房屋,2000多名民眾被迫疏散。電視畫面顯示,佐用全町淪為水鄉澤國,洪流沖 倒樹木,車輛和家具散落在街道上。日本自衛隊派出百餘名官兵前往佐用町救災,但當地仍有8人下落不明。兵庫縣風災罹難者已達12人,附近岡山縣的美作市有 1名68歲婦女因土石流淹沒房屋而喪生。

印 度 和 日 本 差 不 多 同 一 時 間 , 發 生 強 烈 地 震 , 並 且 都 要 發 出 海 嘯 警 告 。

兩 次 地 震 在 本 港 時 間 凌 晨 4 時 左 右 發 生 , 印 度 的 強 度 是 黎 克 特 制 7.6 級 , 震 央 位 於 東 部 安 達 曼 群 島 。

3. The activity of daily record

7:00 AM -- wake up and take a bath
7:20 AM -- breakfast
7:50 AM -- defecation
8:10 AM -- go to school
8:40 AM -- 15:00 PM study at school (12:30 PM lunch time)
15:30 PM --- go home and have some desserts or some cold drinks
16:30 - 17:30 -- exercise ( twice per week )
18:20- 19:00 PM --- dinner time
19:30 - 22:00 PM -- do home work and play computer game a little while
22:00 ~ 23:00 PM -- go to bed (sleeping)

4. Regarding the individual constitution and life style after diagnosis confirmation , we take some life advice as follows:

a . Proper eating habit and a balanced diet
eating style : slight fast , hot /cold mixed , play computer after eating
- suggestion : try to eat slowly first and increase chew timing during eating period
- balance diet : do not intake more cold drinks/foods in order to prevent cough getting worse .

b. Prpoper activities and sleep

- suggestion the lateral posture of sleep(face toward right hand) will be helpful and healthy , on the other hand ,
the life day activity still regular and normal

c. Proper lifestyle in current season ( eg. Winter)
  • It is advisable to eat more food with bitter flavors and reduce salty flavors, in order to reduce the work load of
the kidneys.Since eating too much hard, glutinous, uncooked and frozen food damage the spleen and stomach
  • Winter is cold; and many living things slow down to save energy while some animals hibernate. It is the time
when yang qi becomes latent and yin qi dominates. It's the season when man conserves energy and builds
strength as a prelude to spring. People now have hearty appetites and a lower metabolic rate, enabling absorbed
nutrients to be stored more easily. Appropriate greasy food should be consumed in order to retain warmth.
According to the five elements theory in TCM, winter corresponds to the kidneys which are highly active in winter
and its astringent properties have an active storage function that help save energy.

d. Proper posture and exercise

keep your spine aligned and prevent unnecessarily bent or twisted postures, all of which can contribute to your comfort when standing,
sitting or moving around. Typing can be hazardous to the delicate tendons of your wrists: a correctly shaped keyboard can prevent
carpal tunnel syndrome and associated pain.

Pain is a symptom of postural misalignment and muscular dysfunction, how to correct posture as follows :

  • Is there more weight on the left or right foot?
  • Does the weight bare on the inside or outside of your feet?
  • Are you back on your heels or on the balls of your feet?

Sugestion :
The first thing is to put your feet straight and only hip width apart (4-5" or 10cm). Keep you stomach relaxed and continue to breathe from
you abdomen. Now do the following:
  • Pinch your shoulder blades together - moving them down and back - and release. Repeat X30
  • Make BIG circles with your shoulders forwards. Repeat X30 and X30 Backwards
  • Now squeeze and release your bum (or butt) cheeks. Repeat X60

  • Can't put together three word sentences starting with "I feel..."
  • Can't tell you why she feels the way she does, or can't do it without blaming someone else.
  • Attacks, blames, commands, criticizes, interrupts, invalidates, lectures, advises and judges you and others.
  • Tries to analyze you, for example when you express your feelings.
  • Often begins sentences with "I think you..."
  • Sends "you messages" disgused as "I feel messages" For example, "I feel like you ...

f. Personal development in mind and spirit
  • Due to the client have fire elements activity predominantly leads to consume over qi , in the meantime , lung qi still deficiency as well ,
depends on Five element theory of TCM
  • , Fire(Heart) will overacting metal(Lung) , thus cough will getting worse in night time(yin predominant)

  • Suggesting:
keep calm down 30 minutes when coughing , in order to regulate and smooth the flow of qi at lung
Do exercise of ba duan jing , it can promote the qi and blood circulation in the lung meridian,if possible .

g.Health maintenance by TCM regarding medical history and Lab. report , it is better if you can make a short-term ,
medium and long-term advice e.g. Acupuncture , Moxibustion ,remedial diet.
Due to the client have no Lab. report as this matters, according to the symptoms remaining , make a definite advice for him :
  • short-term advice(acupuncture): select the acupoint to treat his chief complaint symptom(cough)
  • medium advice (remedial diet):to provide the databases available of Energetics of Vegetable / Fruits , A knowledge of food energetics can deeply
supplement a practitioner's ability to help their clients and this article sets out to provide guidelines for giving dietary advice and working successfully
with food(see chart A , chart B)

5.What actions will you take to promote the compliance of your client with your advice on the individual's life style ?
  • Proper diet : to follow the remedial diet to prevent illness is the best treatment ,remedial diet for individual's constitution, taste and flavour ,
minimum side effect and toxicity, convenience.
"Let food be your medicine and medicine be your food" , Healthy eating and keep balance your diet .
  • Regular activities (from day and night)to keep moving but do not overstudy, overwoking to prevent consumption of yang qi (especially in winter season).
  • Purify the mind ,spirit to control desire and follow the simple lifestyle (yin - yang balance)
